XenConvert fails to convert local disk to Provisionsing Services vDisk with "Failed to convert!"
Issue
When attempting to convert a local disk to a Provisioning Services vDisk, the conversion process may fail with "Failed to convert!".
The log shows the following error:

Automount is enabled, despite the above suggestion.
Resolution
Disable antivirus software or manually format the destination Provision Services vDisk before running the conversion.
